PLAZA DENTAL CENTRE – WEBSITE TERMS AND CONDITIONS

Cancellation Policy
We understand that unplanned issues can come up and you may need to reschedule an appointment. If that happens, we respectfully ask for scheduled appointments to be rescheduled at least 24 hours in advance. Our dentists want to be available for your needs and the needs of all our patients. When a patient does not show up for a scheduled appointment, another patient loses an opportunity to be seen.

There is a fee of $50.00 (+GST) for single appointments that are cancelled within less than 24 hours or you do not attend your appointment. [Note: The fee for cancelling double appointments i.e family appointments without adequate notice is $100 (+GST)]. This fee needs to be paid before booking your next appointment and is not claimable with Medicare or health funds.

Thank you for being a valued patient and for your understanding and cooperation as we institute this policy. This policy will enable us to open otherwise unused appointments to better serve the needs of all patients. If there are any circumstances beyond your control which prevent you from cancelling your appointment with sufficient notice you should let us know as soon as possible.

Booking Policy
For appointments longer than 1 hour we require a $50.00 holding deposit. This is because we allocate staff and resources specifically for your appointment. Your booking deposit is credited towards the cost of your appointment, or held as a deposit for future appointments.

Health fund Claiming
If you have dental coverage through your health fund, we can claim electronically through HICAPS on your behalf to help obtain any reimbursement due to you. However, we remind you that your specific policy is an agreement between you and your health fund. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated.

Electronic HICAPS claiming must be on the same day the service is provided, so if you forget your card you will need to pay the full account and then contact your health fund for your rebate.

Payment Policy
Payment of accounts needs to be settled on the day you receive treatment. There may be procedures that are done over two or three appointments. Payments may be broken up in those instances to be paid over the course of treatment. This must be arranged with the Dentist and/or Practice Manager. Payment plans are available for procedures over $250.

Dental Records
Dental records belong to the practice where they are or were created. However, it is recognised in law that patients are entitled to access to their health information.

When seeking to obtain copies of your dental records the following applies:
The request should be made in writing to the dentist or the practice that holds the records. The written request should clearly state:

  • Your name, date of birth and address;
  • That you wish to access or obtain copies of your records;
  • Which records are being requested (e.g. treatment notes, x-rays, etc);
  • How you would like to receive them (digital or hard copy/ pick up in person or post or email); and
  • If you would like the records sent to a third party (e.g. another dentist or health professional) you would also need to provide full details of the third party.

Before providing your records Plaza Dental Centre must verify that the request is genuine and that your privacy and confidentiality is protected against unauthorised access.

At the discretion of your treating dental practitioner, the dentist may request a meeting during which time they can also explain the records. Depending on your circumstances you may wish to meet with the dentist. This is entirely based on your situation and preference.

Dental Records Fee
A small fee of $40.00 will be charged for copying and providing your medical records to yourself or a third party.

New Patient Offer

New patients will receive a free dental examination. The dental examination includes oral exam, x-rays, and treatment plan. Dental records belong to the practice where they are or were created. When seeking to obtain copies of your dental records please read our Dental Records policy.
